Documentation required for subcontract approval:
1. Federal funded projects:
- Fully executed and signed subcontract agreement including pay items and total value;
- Fully completed and signed Form FR-2200;
- Current and unexpired Certificate of Liability Insurance for the subcontractor;
- Current and unexpired Wyoming Unemployment Insurance Certificate of Good Standing for the subcontractor;
- Current and unexpired Workers Compensation Certificate of Good Standing for the subcontractor or proof of Workers Compensation Coverage for out of state contractors; and
- FHWA-1273, either as part of the subcontract agreement or as a separate document upload.
2. State funded projects:
- Fully executed and signed subcontract agreement including pay items and total value;
- Current and unexpired Certificate of Liability Insurance for the subcontractor;
- Current and unexpired Wyoming Unemployment Insurance Certificate of Good Standing for the subcontractor; and
- Current and unexpired Workers Compensation Certificate of Good Standing for the subcontractor or proof of Workers Compensation Coverage for out of state contractors.
Most common reasons for disapproval of a subcontract request:
1. Requested Form(s) DBE-2 have not yet been received and approved for the WYDOT Civil Rights DBE program.
a. WYDOT cannot approve any subcontract requests for a project until all DBE-2's for DBE contractors committed to at time of letting have been received and approved.

2. Subcontractor name on documents is incorrect. All subcontract documents uploaded as part of the subcontract request must be in the name by which the subcontractor is registered with the Secretary of State (either the Wyoming Secretary of State or the Secretary of State for the subcontractor's home state).
3. All necessary and required documents have not been uploaded and submitted with the subcontract request.
4. Dollar amounts furnished on the subcontract agreement and the subcontract request (items tab) are not equal to each other. The dollar amount shown on the request and the dollar amount shown on the subcontract must be equal to each other.
5. Signatures are missing. All necessary and uploaded documents requiring signatures must contain the signatures of both the prime contractor and the subcontractor.
To haul material only on a WYDOT project as an Owner/Operator in lieu of a subcontract:
Subcontracts are WYDOT's prefered method of asigning work on projects.
The Prime Contractor only will decide if a lease agreement option will be allowed for their projects.
- Must be one truck with a single driver with no other drivers or employees.
- Must have a signed lease agreement with the leaser carried in the truck at all times on the project. Lease agreement must also be sent to WYDOT field office.
- Have a DOT letter of operating authority in the truck at all times.
- Carry commercial general liability insurance ($1,000,000 per occurrence and $2,000,000 aggregate) and business auto (minimum $500,000 single limit).
- Visible signage displayed on the truck naming the leaser while on the project.
- Owner/Operators must be listed on the Prime/or leasing contractors weekly certified payrolls.
